Medical Records
To acquire a copy of your medical records, complete a medical release form. The form is available here.

Your records can be mailed, emailed or faxed to you or your healthcare provider. Please indicate where and how you would like the records sent on your release form.
You can also stop by and pick them up in person. Processing time is needed for medical records requests, so records may not be available the same day they are requested.

1-5 sheets = no additional charge |
3-7 days = $0 |
6-25 sheets = additional $10 |
26-50 sheets = additional $15 |

51-100 sheets = additional $25 |
Records sent to another physician's office are free of charge as are records up to 5 sheets, if requested the first time of visit.
